Once you and your trainer have agreed a placement, email the scheme administrator and programme director to confirm your intentions, stating the name of the practice and your trainer. This is important as some practices have more than one trainer and you need to be linked to a particular trainer who will then have access to your departmental information. GP training at Abersychan
Abersychan has a long tradition of providing GP training for registrars based in North and South Gwent. For a number of years we have been able to accommodate two registrars at the same time. Despite the practice having five GP trainers, all members of the primary health care team are involved in GP training.
Regular educational meetings take place “in-house” and the practice is preparing for “Fellowship of the RCGP by Assessment”. A number of partners are completing their personal professional development portfolio.
GP registrars have on average 3 hours protected teaching time per week. Particular interest within the practice includes:

GP registrars are able to call upon the experience of two RCGP examiners in preparation for the MRCGP entrance exam.
We now have 5 partners who are trainers in addition to two who were trainers for a number of years. We regularly have medical students and have taken part in the F2 pilot and nMRCGP pilot studies.

GP registrar duties
GP registrars are a valued part of our practice team. A typical weeks duties are presented below
Each surgery is booked at no less than 10 minute intervals for a maximum of one and a half hours. At the end of surgery all doctors may need to do house calls. One afternoon per week the GP registrar shares the on-call with a partner. Saturday morning surgery is shared with a partner. The Practice uses Deputising Services for out-of-hours cover, but the Registrars are exposed to an adequate amount of experience in managing General Practice emergencies under supervision as stipulated in the regulations.

One day a week will be taken with on call: the afternoon surgery will be for urgent cases only. The day will rotate on a weekly basis i.e. Monday to Tuesday and so on.
TEACHING TIMETABLE
The first 2-4 weeks of the attachment is taken up with an induction programme during which the GP Registrar will spend time with all members of the Primary Healthcare Team. This will include sessions with all the 8 partners in the Practice. Following this period the GP Registrar will have a regular weekly joint surgery with the Trainer, as well as daily random and problem case analyses. There is also a formal weekly tutorial – the topics chosen will be dictated by needs analysis assessments.
Concurrently the Registrar will undertake an audit as well as video analysis of their consultations.
There is an on call commitment with a Partner once a week (Monday to Friday). This will give the Registrar excellent experience in dealing with acute medicine.
During the 6 or 12 months, there will be opportunities to participate in Minor Op, Diabetic, Ante Natal, Infant & Developmental and Acupuncture Clinics. There will also be sessions with the Chiropodist, Dietician, Clinical Nurse Specialist in Palliative Care, Treatment Room and District Nurses.
Each Thursday afternoon the Registrar will be expected to attend the Gwent & South Powys Postgraduate Course at the Royal Gwent or Nevill Hall Postgraduate Centre.

In these days of modern practice, many GPs strive to be GPwSIs (GPs with special interests). We are the original of the species! All GPs have additional qualifications and provide specialist services for patients.
The practice has always had close links with Brecon War Memorial Hospital, our local community hospital. We are proud to look after our own in-patients there and to provide hospital services in cardiology, gastroenterology, anaesthetics, general surgery, gynaecology and palliative care. We also cover the hospital minor injuries unit and support for the emergency nurse practitioners. There are regular out-patient clinics in all specialities which provide you with an excellent opportunity to learn about most areas of practice.
We are a large team of 9 partners, 2 salaried GPs and a large complement of attached administrative and reception staff, practice nurses and health care assistants. The extended team of district nurses and health visitors are all based on site.
We work well as a team and maintain our sense of humour and enthusiasism.

Where are we: We are the closest Gwent practice to Cardiff.

Cwmbran Village Surgery is situated in the centre of Cwmbran less than 20 minutes drive from Cardiff and 5 minutes drive from the M4. It is within easy reach of both Postgraduate Centres, based at Neville Hall Hospital, Abergavenny, and the Royal Gwent Hospital, Newport. Our practice area is largely confined to the town of Cwmbran and its surrounding villages. It enjoys the benefit of a large parking facility, and this combined with its compact geographical area, ensures easy access for our patients and a short journey time for any home visits.
GP training at Cwmbran Village Surgery
Cwmbran Village Surgery has enjoyed the process of facilitating the training of ‘Tomorrows Doctors’ since 1995. In addition to our involvement in GP Registrar training we have provided a stimulating educational environment for ‘GP Specialist Registrars’ and Undergraduate Medical Students from years 3 and 5. Though there are three ‘designated trainers’, all partners are actively and keenly involved in the education process. It will be seen from the short CVs of the partners that all are actively involved in the education process from undergraduate level through to ‘continuing professional development’ and each has there own specialist area of strength thus providing the opportunity for a good ‘all round’ educational experience.
It must be emphasized that the education process extends far beyond that provided by the partners alone and the ethos of continual learning pervades the whole of the primary health care team. Registrars joining this primary health care team will also notice the benefits of working with a stable and skilled workforce that maximizes the benefits of a multidisciplinary approach to patient care in an atmosphere that is relaxed and enjoyable. One of the underpinning philosophies of this practice is that ‘Education is Fun’ and it is a recurrent observation of many of those that have been involved in this practice. Consequently, many who have come to us for their first six months of training have requested to return for their second six months and some have even chosen to stay for good!

Dixton Surgery is a busy, dispensing practice situated on the edge of Monmouth. We are a border practice, so have patients and secondary care providers in both Wales and England. We are a rural practice so have a large practice area covering a large part of the beautiful north Monmouthshire countryside entering into Herefordshire and Goucestershire along the River Wye.
Our practice has a growing population of 3,700 patients, with an interesting mix of town and country, the young and old, the wealthy and the poor. We are high QOF achievers, we offer a wide selection of Local, Directed and Enhanced Services. The Practice shares the clinical leadership and medical staffing of the local hospital.
The Practice has three Partners. Dr Brian Harries is the Senior Partner and Trainer. He has been with Dixton Surgery since 1992 and his special interests are Sports Medicine, Drug Abuse and CHD. His two part-time partners are Dr Shiu Chan who joined the practice in 2006 and Dr Lois Harris who joined in 2008. Dr Chan's special interests are Child and Adolescent Health including Special Needs and Women's Health which are also Dr Harris interests as well as Diabetes.
Both Dr Chan and Dr Harris are are tutors for Medical Students.
Our premises are small and cramped but are well planned and up to date and the Registrar has their own consulting room. The practice uses Emis PCS.
Registrars are welcomed as a valuable member of the clinical team and are expected to provide general practitioner services alongside the Partners, Nurse Practitioner and Practice Nurses. Dr Harries ensures that the pace and content of the training is appropriate, and provides guidance and encouragement throughout the placement. Those with special interests are encouraged and supported with development.

We are located just off the M4 between Newport and Chepstow. Magor is only 20min drive from Cardiff and Caldicot 30min.
We have 20,000+ patients in a semi rural area on the doorstep of the beautiful Wye valley. We have a very mixed population which will allow you to see a range of patients.
Training at Caldicot
We have 3 ex trainers still working in the practice and 2 new enthusiastic Trainers who will organize the Training within the practice. You will have and introductory period followed by surgeries with plenty of consulting time initially, which will be gradually reduced to 10min appointments.
There will be regular protected teaching time within the practice.
Each surgery you will have an allocated GP who will have time to review any problems you may have.
Reviews of your surgeries to aid with your development will regularly occur.
You will have access to the internet at each PC a well stocked library video equipment and be allocated your own room!
We also have Final year Medical students regularly attached to the practice.

BRIEF HISTORY
The Grange Clinic is the oldest established practice in Newport. It was founded in 1924 by the late Dr Henry Anthony Keane, who started his practice from his home at 77 Malpas Road, Newport.
The purpose built clinic at Westfield Avenue opened its doors to receive the first patients on the 1st of July 1989.

Dr Taylor qualified in Belfast in 1990 and trained in General Practice in Northern Ireland and Newport. He joined the practice in 1996 as a replacement for the late Dr Alan Davies.

The Grange Clinic has an extensive practice library with resources relating to all aspects of General Practice, Healthcare and the NHS. These resources are updated regularly.
The NHS Net and Internet are accessible from every computer in the practice including Electronic BNF, Oxford Clinical Mentor and electronic Oxford Textbook of Medicine.
The practice prides itself in the way in which if has embraced Information Technology and is virtually paperless utilising the Emis software. To support the IT system we have a member of staff with special responsibility for IT Training.
We aim to offer prospective registrars a comprehensive package of learning opportunities with emphasis on the assessment of each individual’s differing needs.

Old Station Surgery is a large six partner practice in the market town of Abergavenny on the edge of the Black Mountains and Brecon Beacons National Park. We have 10,500 patients with a good age and social class mix, are part dispensing and the practice area covers about 150 square miles of beautiful countryside.
There are excellent local amenities including hill walking and trekking; Abergavenny also has very good rail and road links and is within easy commuting distance of Cardiff, Newport and Bristol.

Educational Opportunities
We are an energetic and innovative practice. As you can see, the partners have varied interests which will provide registrars with the opportunity to gain experience in these different areas of patient care.
We have an excellent primary health care team who also have a broad mix of skills and experience. Our District Nursing Team have special interests in palliative care and wound management and are also involved in teaching other health professionals.
We are happy for registrars to attend any hospital clinics in areas where they may feel they lack experience eg. Dermatology, Ophthalmology. The whole team – Doctors, Practice and District Nurses, Health Visitors, Counsellor, Chiropodist, Midwives all work from the same building which is modern, well equipped and purpose built. Registrars have their own consulting room.
The Doctors all meet up daily for coffee which is a good opportunity for informal learning and encourages registrars to feel part of the team. Sandwiches are provided for lunch every day!
We have a weekly clinical meeting to which all members of the team are invited. This may consist of a short clinical presentation by an external speaker, or one of the team, or the presentation of an audit or significant event.
We are a paperless and fully computerised practice and have a new practice library/meeting room which has all the usual IT facilities, internet access and CD-Roms’s.
Educational Philosophy
We endevour to provide every registrar with opportunities to meet their learning needs in an informal and relaxed environment.
All the partners are committed to teaching and participate regularly in tutorials and case reviews. As we meet up daily hopefully any problems or concerns can be dealt with as they arise. Rhymney is situated 10 mins from the beautiful Brecon Beacons. Rhymney Health Centre is in the centre of Rhymney town, is situated within half an hour travelling time from Cardiff, Newport, Swansea and Abergavenny. It is within easy distance of the post graduate centres at Prince Charles Hospital and Nevill Hall Hospital.
The Practice serves a population of 6200 in Rhymney, Pontlottyn, Abertysswg and Fochriw. It is a geographically very compact area with few outlying patients which makes home visits a lot more manageable. We operate out of 2 sites, a trust owned building in Rhymney and our own branch surgery in Pontlottyn; but there are plans in progress to build a Resource Centre encompassing a 12 bed inpatient ward with extensive facilities for the GPs and multidisciplinary teams including podiatry, occupational therapy, physiotherapy, optometry and dentistry.
At present we also cover Redwood Memorial Hospital situated in Rhymney a 21 bed community hospital an invaluable resource enabling us to manage more complex medical problems and provide palliative care services.
It is a busy practice with plenty of scope for clinical experience but maintains a friendly and informal atmosphere. We organise regular social activities for staff and students on the premise that "all work and no play...!" The emphasis is very much on teamwork with both our Practice Nurses running minor ailment clinics and the Health Care Assistant holding phlebotomy, spirometry, flu and chronic disease monitoring clinics.
Working here is the closest to "traditional" general practice that you are likely to encounter anywhere with strong links to the community offering the opportunity to reflect on the social economical impact on patients health care needs.
The population is very stable so we provide a truly cradle to grave service whilst still achieving above average QOF performance.
EDUCATION
The practice is strongly committed to education and the pursuit of personal and professional development. We have an extensive library with computer/internet access in a private study room in Pontlottyn surgery.
We have trained 5th year medical students for the past three years and have been delighted with the positive feedback on the standard of education, clinical experience and that they have enjoyed their placement.
Regular MDT meetings with practice and district nurses, palliative care and mental health teams are held and monthly partner/practice meetings all of which students/trainees are encouraged to attend.

Practice Profile
The attractive villages of Risca and Crosskeys nestle in the lower Ebbw Valley, north west of Newport. They are both settled communities, where the main employers are the steel plant at Llanwern, and light industries.
Risca Surgery consists of eight partners, of which two are trainers-one male and one female . We also take medical students from both Cardiff and Royal Free Hospital medical schools.
The practice occupies a cost-rent building completed in July 89.
One of the partners is an ex medical director of the LHB.
We undertake all the usual clinics, e.g. child surveillance, minor surgery, diabetic, asthma and hypertension. A recently built extension to the treatment area has enabled us to increase the number of out-reach clinics we hold at our surgery, including sigmoidoscopies, counselling, and orthopaedics. The treatment room is also fully equipped so we can undertake a wide range of investigations, and procedures ourselves. There is also the usual access to X-ray, pathology, and physiotherapy services.
We have attached staff e.g. Midwives, Health Visitors, and District nurses who are based in our surgery.
We are fully computerised and have a well stocked library. In the library there is also a computer terminal for teaching purposes which is connected to the Internet and Medline as well as Internet access on our desktop. We also have both video recording and play back facilities which are used for teaching.
On-call is shared with another local practice , being approximately 1:10 , with deputising taking over at midnight and covering weekends. On-call accommodation is available if required.
The practice philosophy on teaching is that the teaching should be structured towards the needs of the registrar and hopefully the hospital component of the training can be more tailored towards the needs of the future general practitioner. We feel that a lot of the consultant based training can be undertaken from within the confines of the practice itself.
We feel the registrar is in the practice to learn and gain experience in all areas of general practice and is not seen as merely an extra pair of hands but becomes an integral member of the primary health care team.

Training at Tudor Gate Surgery
Tudor Gate Surgery has a long history of being a training practice. All members (See Below) of the Primary Health Care Team take part in training.
Dr. Plumb and Dr Bracchi are Trainers and Dr. Chidgey is soon to be approved as a Trainer. Tudor Gate Surgery is allocated one Registrar at a time and the Registrar has their own room for the duration of their allocation.
The Registrar on first entering the practice has an induction period where they can learn about all the roles of the Primary Health Care Team. The Registrar will be allocated to one Trainer for their stay at Tudor Gate. Teaching is done weekly by the Trainer and also there will be a second tutorial done in rotation with the other Doctors in the practice. Throughout their stay the Registrar is invited to attend Practice meetings, Monday 8.30am Business meeting, Tuesday 1.30pm educational meeting and Wednesday 1.00pm Primary Health Care Team meeting.

The Community Nurses specialize in providing nursing care at home to the chronic sick disabled and terminally ill. They hold wound and bladder dysfunction clinics at the surgery. They can be contacted on 01873 850361 on weekdays between 9.00 am and 5.00pm
Health Visitors

The Health Visitors are based at the surgery also and are responsible for maintaining the health of the pre-school children and the elderly population. They assist the Primary Health Care Team with childhood immunizations and where necessary pay domiciliary visits to the homes of the under 5's and the elderly. They run the Well Baby Clinic on Monday afternoons between 2.00 and 3.00 p.m. Immunization and health checks are done by appointment. Pre-retirement checks, teenage clinic and over 75 checks are also available.
Midwives
The Midwives provide the majority of ante and post natal care. The Ante-natal Clinics held here in the surgery by appointment on Wednesday afternoons. They also provide care at the home where necessary, and will routinely visit new mothers and babies at home for 10 days.
Community Psychiatric Nurses (CPN’s)
CPN’s are attached to the practice and work closely with all members of the Primary Health Care Team. Patients are seen by appointment.
Counselling Staff
The surgery is happy to provide a counselling service by G.P. referral.

Mr. Kirsten Jones is responsible for the day to day running of the practice and supports the other team members and doctors.

We have a very friendly team who will be happy to help the Registrar though out their stay.
The Computer
We use Torex Synergy computer software. All consulting rooms have access to the system.
Non-NHS Services
Some medicals are not covered by the NHS e.g. HGV Insurance and pre- employment medicals. We are pleased to provide these services but they incur a charge. The practice secretaries can provide details.
Most Travel Vaccinations are outside of NHS services.
Occupational Health services are not our responsibility and unfortunately we are not able to do these even privately.
Temporary Residents
Everybody resident permanently in the UK is entitled to NHS treatment at a general practice. A British National living abroad is now treated privately and can claim back on their insurance.
Consultations
Consultations are by appointment by either telephoning or calling at the surgery. The appointment diary is usually available to up to four weeks ahead. An appointment is for one person only.
Dispensing Services.
We are a dispensing practice which is available for rural patients only. You have to register with us to be able to get your medication direct from us. Ask at reception if in doubt.

The practice is located in the village of Trethomas outside Caerphilly and is only 15 minutes from Cardiff and Newport – the closest to Cardiff in the Gwent VTS scheme. The practice population is approximately 10,500 patients and we cover the villages of Machen , Lower Machen, Draethen , Rudry, Waterloo, Trethomas and Rudry.
We believe we are a forward thinking practice which readily embraces changes in general practice. We have an active Primary Health Care Team including a Salaried GP, 2 Nurse Practitioners and 2 Health Care Assistants. The practice runs weekly Chronic Disease Management Clinics in CHD, Diabetes and respiratory disease. Family Planning clinics and Minor surgery sessions also take place on a weekly basis.
Education
The practice is involved in teaching 3rd year medical students . We hold monthly educational meetings including practice audits and Significant Event Analyses. All doctors have desktop internet access and access to the NHS Wales intranet.

Practice appointment times are extremely flexible and the registrar will be able to choose start and finish times to suit themselves. Appointments are of 10 minute duration

Salaried GP – Marilyn’s interests include paediatrics, women’s health and palliative care. Marilyn has developed the palliative care services in the surgery and is an excellent resource for this potentially difficult area of general practice training.

The Medical Centre was built in 1991 and is situated some 5 miles from Newport and 3 miles from the M4 (Junction 28), making it easy to access from Newport and Cardiff. Training has been carried out in the Practice for over 35 years.
The list size is about 6500, the majority of patients living within a 2-mile radius of the Surgery. With only four partners our Registrars tell us that they particularly enjoy the informal and friendly atmosphere of a small practice.
Although Risca is an urban area there are fine views of Twm Barlwm Mountain to the north, and the more gentle green fields and woodland to the south give a rural feel to the town.
New housing means we have many patients who work in Newport Cardiff and Bristol, but on the whole there is a closely knit village atmosphere.
Personal and continuing relationships are considered important at Wellspring. The registrar will need to have three skills vital to General Practice; tolerance, friendliness, and an ability to chat, the rest we can usually teach!
The Surgery has patient and staff charters and is actively involved in Total Quality Management.
Records have been kept with the EMIS Computer system since 1995 making us virtually paper free. Registrars have found the transition to electronic record keeping painless as we employ a large number of methods to ease the inputting of consultation records. Letters are scanned into the computer for easy reference. EMIS (Version 5) allows electronic access to the BNF as well as Mentor (the Oxford Handbook of Clinical Medicine). The Practice is now using PRODIGY, for decision support in prescribing. Patients can book there appointments online.
Appointments for patients are at 10 minute intervals. Morning surgeries last from 9 am until about 12 noon. Afternoon surgeries are from 3 pm until about 5:30 pm. We endeavour to see patients at their appointment time and are usually able to offer even non urgent appointments on the same day.
There are 2 Practice Nurses each with special areas of interest There is also a practice counsellor.
The Registrar will be actively involved in all areas of the Practice. The initial induction period lasts for about one week (depending on the confidence of the Registrar).Close supervision follows with all cases being reviewed until the end of August. After that there are regular tutorial sessions with protected time with all of the Partners as well as daily informal team meetings over coffee, which the Registrar attends.
The Registrar will receive a Registrar handbook giving useful information, training objectives, learning style and attitude questionnaires and details of summative and formative assessment. Learning plans are developed after assessing each Registrars needs.
Registrars are encouraged to use video recording as an aid to learning instead of just being an element of MRCGP and Summative Assessment but we do fully understand that some Registrars are not very comfortable with being videoed so we try to make this a pleasant an experience as possible! Registrars are encouraged to sit the MRCGP and other Postgraduate examinations.
The Library is well stocked with journals, books and other reference material. Internet access allows searches on Medline and other medical databases. A number of CD based educational materials are also available
